OUR OFFICE
On your first visit to our office, we will introduce you to our practice and you’ll meet Dr. Smith and our caring staff.
Dr. Smith will review your medical history, discuss any pertinent information such as symptoms, perform a comprehensive examination, and review any diagnostic studies you have already had, so it’s important to bring them with you. You should also bring your completed new patient forms, identification, insurance card, physician referral (if any), and any medical records pertaining to your office visit with Dr. Smith.
At the conclusion of your visit, Dr. Smith will formulate a treatment plan based on your condition. If you have any questions, don't hesitate to call our office at (732) 875-3814.
